Refer to SB104 section 1 on page 2. Please do the following if
your extendo acme nut mount is a two-bolt design as shown in the
figure below:

Mounts
Replace the two bolt extendo
power adjust mount with a four bolt mount. The mount should be
installed with four grade 5, 3/8” UNC bolts and lock washers. The
Acme rod plastic nut fits inside the mount and the mount cover and
the mount are bolted to the extendo frame. Torque the bolts to 33
ft/lbs.
