Description of Problem Proper Procedure Enclosures

To our valued customer:

This service bulletin is part of an ongoing effort to provide quality service and support for all Alliance Machine Systems International, LLC products. If you need assistance or more information, please call Gary Kneale, Field Service Manager, at 509-535-0356 and select Option two.

Description of Problem

A serious injury has occurred on a TEI Block Push Bottom Feeder that apparently was the result of an operator entering the lift area hazard zone without using proper lockout procedures.

Proper Procedure

Proper lockout procedures should be followed any time personnel enter a hazard zone.  The primary hazard zones on the Block Push Bottom Feeder are:

  • The lift area inside the main frame, extending to the back of the block pusher and the separation conveyor.

  • The area underneath the Extendo conveyor, including the tamper travel area.

Personnel should NOT go into these hazard areas unless they have completed the “Hazardous Energy Lockout Procedure” listed in the operator manual.

Please take time to reiterate the importance of using proper lockout procedures with your personnel.  Please contact Alliance if you need additional copies of the operator or maintenance manual.
