Description of Problem

We have identified a potential problem with the pusher assembly on the ASC/Pallmac Prefeeder. There was an incident where a weld failed on the cross tube weldment, allowing the pusher assembly to drop down onto the lower conveyor.

Recommended Solution

The weld on the pusher assembly should be inspected for visual cracks, paint chips, or signs of material deformation.  The weld should also be checked to ensure that it conforms with the weld symbols as indicated on the attached weldment drawing.  Any welds that are not to specification should be repaired to the drawing specification by a certified welder.
