Description of Issue
Alliance has learned of an incident in which a prefeeder operator recently tried to grab the Extendo safety leg lockout pin while it was retracting causing a minor injury.
The Extendo safety legs contain an air-operated pin that positions the lockout pin through the safety leg. The lockout pin has a small-diameter hole bored through it to allow an individual lock or a gang lock to be installed to comply with some customers’ safety policies. The lockout pin retracts through the leg during normal operation to allow the Extendo to raise/lower into position. A representative picture is shown below (your machine may look different due to generational design changes).

Discussion
Alliance had designated the area as a hazard area and informed the operators and maintenance personnel through training, decals and manuals that personnel should not enter the area unless the machine is locked out. The following photograph shows a warning decal and an informational decal in the vicinity of the extendo lock out leg/pin.
