Description of Issue

The range of motion of the pusher frame inside the CUBIX is approximately 6”. When the machine is powered off, the frame is held in place by a pilot-operated load-holding check valve. In the case of check valve failure, the frame can drift to full-down position. If maintenance or operation personnel are inside the machine when this happens, they could become pinched between the moving frame and other parts of the machine or the ground.

Discussion

Alliance has determined that a modification to the pusher frame hydraulic circuit will allow the pusher frame to lower when the circuit control power is removed, eliminating the hazard. It will take approximately 1.4 seconds for the pusher frame to move from full-up to full-down position when control power is removed; the hazard still exists during this time period. A retrofit package, new hydraulic schematic, and instructions have been developed and provided that will allow maintenance staff to implement the solution. In addition, a warning decal reminding personnel to verify that all machine motions have stopped has been included.

Recommended Action

Alliance recommends that you take the following actions within 10 days after receiving this safety bulletin and retrofit package.

  • Remove the pilot-operated load-holding check valve in the pusher up/down circuit per the enclosed instructions.
  • Install the warning decals next to the handles of the doors into the machine per the instructions.
